Payment in advance by check is acceptable; should you prefer to make payment at pick-up, cash is required. Payment is required in full when services are rendered. We do NOT accept credit cards.
Boarding Agreement
In consideration of boarding services for ________________________________ (name of dog/cat), ________________________________ (breed), I agree to hold Bed & Bones harmless from any and all liability which may occur (with relation to my pet) during the period services are rendered. Related items may include, but are not limited to, administration of veterinary care and any associated costs; loss or harm of or to my pet; any costs should my pet harm another person or animal; replacement of any items which my pet may damage; reimbursement for any additional food; parasite control; or other supplies consumed or needed.
I verify that my pet is current on all vaccinations, in good health and has not been ill within the last 60 days (any exceptions should be noted at the time you make your reservation for boarding and must also be written on this agreement)*. Written verification from your veterinary office is required for proof of rabies, distemper and bordetella vaccinations in dogs, rabies, distemper and feline leukemia in cats. We highly recommend that your dog's bordetella vaccination be LESS than six months old in order for it to have as much protection as possible from the contraction of kennel cough.
Bed & Bones will provide reasonable and responsible care for the above referenced animal to the best of its ability and take necessary precautions to provide healthy and safe accommodations.
With your signature below* you indicate knowledge that you are responsible for your pet and its actions and understand that group play among dogs may result in injury, for which Bed & Bones will be held harmless. Although adequate measures are taken for parasite control, we may not guarantee that your pet will be returned without fleas or ticks.

*Should you email this Boarding Agreement, it shall be deemed the same as a signature with your return email address. Our signature shall be deemed the same as well.
We do not require you to sign a new boarding agreement each time you board your dog/cat with us, but please be aware that these requirements are in effect for any future boarding requirements you may have. The withholding of any relevant health information exposes you to liability for the health and safety of all animals boarding with us during your pets stay with us. With your signature above you also indicate you an understanding of general operations, understand that you are responsible for any costs incurred for the care of your pet and have truthfully disclosed the temperament of your pet.
